(I) Identification

1. Patient

Eg triage officer, HO,MO, Specialist ,referral team

(II) Assessment :
- identify the loss,the problem,risk & severity of the loss
Eg morbidity & mortality
- identify the probability of the problem recurring

(III) Potential risk Mx : 
1. Retention
2. Reduction
3. Avoidance
4. Transparent

(IV) Create risk Mx plan :
- need approve from higher authority
- must be applicable
- plan must be " check & balance "

(V) Implementation :
- try and error
- mock drill
- assess the adequate plan

(VI) Reassessment & review :
Audit
